Patient Confidentiality
We respect you right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible care. This information may be used for management and audit purposes. However it is usually only available to, and used by, those involved in your care. You have the right to know what information we hold about you. If you would like to see your records, Please call the surgery.
When you register certain personal details, Such as name, address, NHS number and date of birth are passed to the NHSE and to the NHS central register. Your medical record can then be located and passed to your new practice. The NHSE database holds certain information such as childhood vaccinations, immunisations and cervical cytology. The NHSE database and the central register do not hold any other clinical information.
It may be necessary to share information regarding your medical history with other health care professionals such as hospital consultants, To ensure you receive appropriate treatment. There are also certain statutory requirements that require a practice to pass on information, ie notification of births or deaths, Infectious diseases and gunshot wounds.
In other cases, Such as releasing medical records to solicitors when dealing with complaints or legal claims information is only released with your written consent to do so.
Access to Medical Records
Any Patient is entitled to seek access to their records. Requests must be made in writing via Keele Practice email address (Docman.m83670@nhs.net). For further information contact the surgery on 01782 753550.
Zero Tolerance
The practice supports the NHS policy of Zero Tolerance which defines violence as:
"Any incident where staff are abused, threatened or assaulted in circumstances related to their work, involving an explicit or implicit challenge to their safety, well-being or health".
The practice takes a serious view of any incidents of physical and verbal abuse against employees and will support them, If assaulted, Threatened or harassed, So long as the employee is correctly carrying out his or her duties.
We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, They will be warned to stop their behaviour. If they persist, We may exercise our right to take action to have them removed, Immediately if necessary, from our list of patients.
Teaching/Training of Health Care Professionals
The practice is sometimes involved in the training process of health care professionals. From time to time you may be asked prior to your consultation if a trainee may be present during your consultation. If you do not wish this person to be present please inform reception before your consultation so that the trainee may leave before you go in for your appointment.
